Careport Health is an online booking engine for post-hospital care. 8 out of 10 patients leaving hospitals need additional care such as at a nursing home or at home through visiting nurse services. Our matching algorithm finds patients the post-hospital care they need when they need it. Additionally, we aim to track patient care to improve communication with family and primary care providers.
